Is the dot exactly where the satellite is?

It is calculated from the public orbit file, which is a few hours old. For something as low as the space station that can be a short distance off. It is close enough to see which part of the world it is over.

Why not every satellite?

There are thousands, most of them faint. This map keeps the ones people ask about: the station, the weather satellites, GPS, and a list of bright ones.
