Tiger attack victim William Chapman: medical records may have been breached

Possible access to records
Yorkshire Ambulance Service NHS Trust said it found two possible incidents involving staff accessing or attempting to access information about Chapman’s case after his death. The trust said the access may have been without a legitimate reason.
A spokesperson said Chapman’s family knew about the potential breach and that the trust had offered condolences. The trust said its proactive audits identified the concern and that it had reported the matter to the Information Commissioner’s Office. The spokesperson said the trust takes patient information confidentiality and security seriously, and would keep the family informed as its investigation continued.
Death at wildlife park
Chapman died on Saturday after entering the tiger enclosure at Yorkshire Wildlife Park. An inquest at Doncaster Coroner’s Court heard on Thursday that he died from multiple traumatic injuries and animal interaction.
After the attack, park staff secured the tiger. Paramedics attended and confirmed Chapman’s death at the scene, the inquest heard.
Inquest hears about care
The inquest heard Chapman, 24, was receiving support from adult mental health services in Doncaster. He was described as highly functioning and independent, with no care requirements.
