A failed Substance revival creates a grotesque hybrid: Elisabeth and Sue’s ending explained, and what happens next
A rivalry over one life
Elisabeth Sparkle, played by Demi Moore, is a former star fired from her fitness television show. She uses a mysterious serum to create Sue, a younger double played by Margaret Qualley. The two share one life, taking turns in weeklong periods and relying on stabilizer fluid to maintain the process.
The company behind the Substance urges Elisabeth to see Sue as part of herself. Instead, the pair increasingly become rivals for their shared time. Sue eventually refuses to give control back, leaving Elisabeth in a deteriorated state.
Mercy gives way to violence
When Elisabeth regains control, she tries to kill Sue but stops after injecting only half the solution. Her horror at what she has done and her ego lead her to spare her younger self.
Sue does not return that mercy. She kills Elisabeth, believing she can now live without the older woman. But without Elisabeth to produce stabilizer fluid, Sue’s body begins to break down.
A failed attempt to survive
Sue tries to reuse the one-use-only Activator serum. The result is Monstro Elisasue, a grotesque hybrid with multiple limbs and Elisabeth’s face protruding from its back. Elisabeth is, in a sense, resurrected, but only as a face trapped on Sue’s body.
As the creature deteriorates, Sue loses her teeth and nails, and her ear detaches. Desperate for love, she puts on a sparkling gown, makes a mask from a photo of Elisabeth’s face and goes onto the live New Year’s Eve stage.
A tragic final image
The audience sees a monster, and Monstro Elisasue flees the show. Its body collapses and self-destructs, leaving Sue effectively dead and Elisabeth with little time left as a face.
The transformation gives physical form to the pair’s increasingly destructive choices. The film treats both as pitiful figures, using their fate to question the obsessive pursuit of control over women’s aging bodies.
